taking the first steps to getting healthy again.

We order the most carb-heavy, gluttonous, ramen deliciousness

And we found out right in the middle of our Disney trip.

So that was fun.

They're with just dads.

to grill and things like that?

Yeah, they coordinate.

Actually, the one guy who's the head of our little

But he brings a giant grill and he makes sure there's

enough food and he does all this stuff, and we just chip

in some money and help clean up and do all that fun stuff.

So I pulled in another David with me here and another

friend of mine, Dave Wylly.

So a little background.

Josh Bretl:

David is a Medicare specialist.

Josh Bretl:

He's an expert in Medicare, and my father, five

Josh Bretl:

years ago was turning 65.

Josh Bretl:

Now, we happen to own a practice that specializes

Josh Bretl:

in working with retirees and Medicare's a hot topic.

Josh Bretl:

Well, we never had anybody who we would send

Josh Bretl:

Medicare referrals to.

Josh Bretl:

It's something that we don't handle.

Josh Bretl:

It's a very specialized thing, and we have always

Josh Bretl:

felt that when it came to Medicare, most Medicare

Josh Bretl:

salespeople who approached us were buying a used car.

Josh Bretl:

It was a dirty, used car.

Josh Bretl:

And it wasn't until, actually it was one of my dad's

Josh Bretl:

high school friends, they just happened to all be

Josh Bretl:

turning 65 at the same time.

Josh Bretl:

Who is a really, really smart guy.

Josh Bretl:

He says, "Hey, you've got to meet this Dave guy.

Josh Bretl:

It's something totally different."

Josh Bretl:

And so, I don't know if you remember, but you came in and

Josh Bretl:

you talked to my dad and I, and we brought you in to do

Josh Bretl:

a workshop and we all said, "This is totally different."

Dave Wylly:

Yeah.

Dave Wylly:

You guys definitely put me through the full nine yards.

Dave Wylly:

That's for sure.

Josh Bretl:

We did.

Josh Bretl:

But every time we've heard you talk, it's like our whole

Josh Bretl:

staff learn something new and it's just a great experience.

Josh Bretl:

So I appreciate all of your help over the years.

Dave Wylly:

It's a complex topic and yet it's not as scary if you break

Dave Wylly:

it down into bite-sized chunks.

Josh Bretl:

We're not going to have a ton of time here because

our podcast listeners get really sore on their butt if we make

them sit and listen too long.

Josh Bretl:

But I wanted to go through a few key questions for you

Josh Bretl:

and then we'll talk about how they can reach out and

learn more as we move on here.

So the person who's listening just turned 64 years old and

Josh Bretl:

on their birthday, they start getting their first batch of

Josh Bretl:

mail and it's every insurance company under the sun.

Dave Wylly:

It's unbelievable.

Dave Wylly:

The landslide, if you've ever owned an insurance product

Dave Wylly:

in your life, and property and casualty, automobile,

Dave Wylly:

homeowners, any of that, they've got your birthdate,

Dave Wylly:

they've got your address, and they probably have your phone

Dave Wylly:

number, maybe your old phone number, but you'll find that

Dave Wylly:

the phone rings off the hook and your mailbox fills up.

Dave Wylly:

It will be unbelievable.

Josh Bretl:

So you can't get Medicare till 65.

Josh Bretl:

This starts at 64.

Josh Bretl:

When should people start worrying about it?

Dave Wylly:

Well, we always say anytime you feel your blood

Dave Wylly:

pressure going up, we do a lot of educational seminars and

Dave Wylly:

we find that people will show up, and you'll run the gamut.

Dave Wylly:

You'll have folks that are in their late 50s.

Dave Wylly:

You'll have folks that have already made their Medicare

Dave Wylly:

decision, and yet that gives me a great source of pride because

Dave Wylly:

they're smart enough to revisit the initial decisions they

Dave Wylly:

made because maybe they didn't make a fully informed decision.

Dave Wylly:

But you see people, will get people coming to our

Dave Wylly:

office that saw us four or five years ago at a library.

Dave Wylly:

So I would always say the first thing you want to do

Dave Wylly:

is start to educate yourself.

Dave Wylly:

Waiting till the last minute is not the answer.

Dave Wylly:

Do not ignore it.

Dave Wylly:

It's not going to go away.

Dave Wylly:

And it's the more you learn about Medicare, the more

Dave Wylly:

you're going to appreciate what a great program it is.

Dave Wylly:

But you just need to understand the intricacies involved.

Josh Bretl:

So I got a few things out of that.

Josh Bretl:

One is don't wait.

Josh Bretl:

So if someone's 60 years old and they're freaking out about

Josh Bretl:

Medicare, they can come watch one of your presentations.

Dave Wylly:

Anytime you feel your blood pressure going

Dave Wylly:

up is when we should meet.

Josh Bretl:

Okay.

Josh Bretl:

Here's a second question.

Josh Bretl:

You're 68, 69, 75 years old, and you made a Medicare choice

Josh Bretl:

at 65 and you just kept the same thing over and over again.

Josh Bretl:

Is there room to be better?

Josh Bretl:

Is there...

Dave Wylly:

There's always room to investigate.

Dave Wylly:

We find an awful lot of people come to Medicare and they

Dave Wylly:

did what their neighbor did.

Dave Wylly:

They did what the guy at work did.

Dave Wylly:

They did what their brother-in-law did, and

Dave Wylly:

that is not usually the best opportunity to maximize value.

Dave Wylly:

And everybody's value equation is a little bit different.

Dave Wylly:

How are you doing financially?

Dave Wylly:

How are you doing physically?

Dave Wylly:

What's your risk tolerance?

Dave Wylly:

What's your medical history?

Dave Wylly:

These things all go into a comfort level.

Dave Wylly:

The problem is most people come to Medicare, they make

Dave Wylly:

one decision, they throw it in a drawer and they don't

Dave Wylly:

ever want to visit it again because it was so damned

Dave Wylly:

confusing the first time.

Dave Wylly:

So what they wind up doing is just ignoring.

Dave Wylly:

And we know, even though there's a landslide of

Dave Wylly:

advertising, every year you get a chance to do your drug

Dave Wylly:

program over or maybe look in an advantage program or

Dave Wylly:

just reassess where you are.

Dave Wylly:

That's from October 15th to December 7th.

Dave Wylly:

And we know studies tell us about 65% of all

Dave Wylly:

seniors never even look.

Dave Wylly:

They got so confused the first time.

Dave Wylly:

They don't even want to deal with it.

Dave Wylly:

And we know the most terrifying word that seniors

Dave Wylly:

can hear here comes change.

Dave Wylly:

Oh my God, they hate that word.

Josh Bretl:

It's not just seniors, trust me.

Josh Bretl:

We're really big.

Josh Bretl:

We want people to be in control and own their situation.

And bearing your head in the sand about Medicare for

your entire retirement is one of those things that

you're just hurting yourself.

Dave Wylly:

That's one of the saddest things.

Dave Wylly:

We'll catch people in their late 70s, early 80s, even

Dave Wylly:

further on down the road because eventually these decisions that

Dave Wylly:

you made, whether you look at it yearly or every 10 years,

Dave Wylly:

it's eventually going to be something that's going to grab

Dave Wylly:

your attention because it's going to continue to increase

Dave Wylly:

in cost on a fixed income.

Dave Wylly:

You're eventually going to bump into the decision point

Dave Wylly:

of what are we doing and is there a better alternative?

Dave Wylly:

And that's one of the great things that we're able to do

Dave Wylly:

is rescue people in their later years that might not have made

Dave Wylly:

smart decisions initially and be able to improve their situation.

Dave Wylly:

Usually it's fairly dramatic, but as always, the saddest part

Dave Wylly:

of that conversation is when you explain to the couple that

Dave Wylly:

with a couple couple of prudent moves, you can save about $180

Dave Wylly:

a month between the two of you and they get very excited.

Dave Wylly:

But it always seems like it's mom that wants to

Dave Wylly:

know how long could we have been saving $180 a month?

Dave Wylly:

And you have to try to tell them, "Well, I'm glad I

Dave Wylly:

didn't meet you at 86 because you're paying way too much."

Josh Bretl:

yeah, in $180 a month, it's real money.

Josh Bretl:

I mean, it's-

Dave Wylly:

Times 12.

Josh Bretl:

Yeah, that's every year.

Josh Bretl:

Let's take a step backwards here.

Josh Bretl:

Medicare is ungodly confusing and that's talking from

Josh Bretl:

someone who knows the tax code.

Josh Bretl:

A, B, D, Advantage, all that fun stuff.

Josh Bretl:

Can you give us a brief description of how to

Josh Bretl:

think about Medicare?

Dave Wylly:

Well, you start with Medicare being primary,

Dave Wylly:

and this is a tectonic shift.

Dave Wylly:

You have to almost check what you learned in your

Dave Wylly:

first 65 years of insurance at the door because now

Dave Wylly:

there's a new sheriff in town.

Dave Wylly:

This is Medicare.

Dave Wylly:

The federal government is going to step into place, and

Dave Wylly:

that doesn't have to be at 65.

Dave Wylly:

You might wait longer if you continue to

Dave Wylly:

have coverage at work.

Dave Wylly:

But the first step in the process is wrapping your

Dave Wylly:

mind around how Medicare breaks out facilities.

Dave Wylly:

That's all under part A, and services, that's

Dave Wylly:

all under part B.

Dave Wylly:

And that's a little bit different, but it's the

Dave Wylly:

buildings that you need is part A, and then everything that goes

Dave Wylly:

on in the buildings is part B.

Dave Wylly:

And what you want to know is that that's really good coverage

Dave Wylly:

at a really good price, but it's nowhere near complete.

Josh Bretl:

So that A and B is government

Josh Bretl:

run, government control.

Josh Bretl:

They set the rules, they set the cost, all of that.

Dave Wylly:

That's the foundation.

Dave Wylly:

You got to have that.

Dave Wylly:

That's the cover charge to get into the wonderland

Dave Wylly:

of Medicare, A and B.

Dave Wylly:

And once you get educated and understand what that does,

Dave Wylly:

and then conversely what that doesn't do, then that opens

Dave Wylly:

the door to a discussion about how private insurance

Dave Wylly:

companies can step in and fill those gaps or supplement what

Dave Wylly:

original Medicare doesn't cover.

Josh Bretl:

So the supplements, those are what

Josh Bretl:

people are getting mail for.

Dave Wylly:

These days, supplements are not

Dave Wylly:

getting as much push as the Advantage programs.

Dave Wylly:

Advantage has become a very hot topic because it features

Dave Wylly:

low premiums and Joe Namath.

And a high commission for the salesperson.

Pays absolutely the highest

commissions in my industry.

And it drives me crazy because if you're doing a analysis of

what folks need that should be, and I know Josh and I share a

similar philosophy, it's called fiduciary responsibility.

Dave Wylly:

Are you doing what's in the best interests of your clients?

Dave Wylly:

And a lot of agents that I've seen in the field and the

Dave Wylly:

advertising on TV is incessant.

Dave Wylly:

Advantage is a gold mine for the agent.

Dave Wylly:

It's a gold mine for the company that's marketing

Dave Wylly:

it, but it might not be your best option for coverage.

Josh Bretl:

And I'm someone who thinks graphically,

Josh Bretl:

I love a good flow chart.

Josh Bretl:

And my family makes fun of me.

Josh Bretl:

But the way you picture it, actually, you have a handout in

Josh Bretl:

the seminars that's fantastic.

Josh Bretl:

And you have two paths that it follows.

Josh Bretl:

You have this traditional path, part A, part B,

Josh Bretl:

and the supplement and-

Dave Wylly:

A drug program.

Josh Bretl:

And a drug program.

Dave Wylly:

We called original Medicare.

Dave Wylly:

That's one pathway.

Josh Bretl:

Okay.

Josh Bretl:

And the other pathway is the advantage pathway.

Dave Wylly:

Correct.

Dave Wylly:

And we'll see a change here probably in the next year

Dave Wylly:

because what we're hoping is going to come out of

Dave Wylly:

Washington DC is they're going to take the word Medicare

Dave Wylly:

off of Medicare Advantage, all these advertisements

Dave Wylly:

on TV intimate that you are part of Medicare when you're

Dave Wylly:

with an Advantage program.

Dave Wylly:

That's not true.

Dave Wylly:

Advantage is a private health insurance alternative, doesn't

Dave Wylly:

have the same catchy phrase.

Dave Wylly:

It's not as catchy.

Josh Bretl:

How you name that is genius.

Dave Wylly:

Yeah.

Dave Wylly:

So what they've got is people being convinced that they're

Dave Wylly:

part of the Medicare world.

Dave Wylly:

You still have A and B, you still got to pay the part B

Dave Wylly:

premium, but with Advantage, you've moved to a different

Dave Wylly:

platform where now a private insurance company is running

Dave Wylly:

your healthcare and there's a market difference between

Dave Wylly:

making decisions with your doctor, and that would be

Dave Wylly:

the pathway called original Medicare, or making decisions

Dave Wylly:

with an insurance company.

Dave Wylly:

That's what Advantage is.

Dave Wylly:

The federal government pays a private insurance company

Dave Wylly:

and they step in and literally replace original Medicare.

Dave Wylly:

Now, everything is run through them.

Dave Wylly:

It's not a bad thing.

Dave Wylly:

But boy, those advertisements on TV drive me crazy.

Dave Wylly:

They're very disingenuous.

Dave Wylly:

The way that they're pitching dental, vision, hearing, get all

Dave Wylly:

the benefits you're entitled to.

Dave Wylly:

Let's take a real good look at those benefits in what you're

Dave Wylly:

actually getting and what you're actually giving up.

Josh Bretl:

So the first step is to decide original

Josh Bretl:

Medicare, that pathway with A, B, D supplements

Josh Bretl:

or the Advantage path.

Josh Bretl:

Is that correct?

Dave Wylly:

Yeah.

Dave Wylly:

Yeah.

Dave Wylly:

I would say when you look at A and B, and we would refer

Dave Wylly:

to A and B, the bedrock, the foundation, that's a sunk cost.

Dave Wylly:

So when you realize that you got to have A and B, then the

Dave Wylly:

way you really analyze whether it's original Medicare or an

Dave Wylly:

Advantage program, you'd want to look at buying a drug program in

Dave Wylly:

a supplement that would complete the original Medicare pathway.

Dave Wylly:

So what is the drug program and the supplement cost as

Dave Wylly:

opposed to an Advantage program?

Dave Wylly:

Where most advantage programs are touting, and it's true

Dave Wylly:

zero premium, you're not paying a monthly premium, but

Dave Wylly:

you're out of pocket exposure is dramatically higher and

Dave Wylly:

your network restrictions are something that need

Dave Wylly:

to be analyzed in depth.

Josh Bretl:

So you'll pay less-

Dave Wylly:

Upfront.

... Josh Bretl:

probably upfront, but they're going

... Josh Bretl:

to limit who you can see, when you can see them.

... Josh Bretl:

And it's going to cost you a lot more when you need it probably.

Dave Wylly:

This is the bane of my existence, is people

Dave Wylly:

who spend 12 minutes on the phone with somebody in a

Dave Wylly:

teleservice agency in Florida and decide to dramatically

Dave Wylly:

change their coverage.

Dave Wylly:

And yes, it's true with Advantage you do get bells and

Dave Wylly:

whistles and shiny objects, but when the rubber hits the

Dave Wylly:

road and you really need health insurance, people are dismayed

Dave Wylly:

to find out they traded free toothpaste for the Mayo Clinic.

Dave Wylly:

To my opinion, you want to be able to see the doctors,

Dave Wylly:

you want to see when you want to see them, what original

Dave Wylly:

Medicare is all about.

Josh Bretl:

Now, we've just spent the last five

Josh Bretl:

minutes bashing Advantage.

Josh Bretl:

I'm sure there are people who is-

Dave Wylly:

Who love it.

Josh Bretl:

Who is in it, and it's appropriate for them.

Dave Wylly:

Sure, could be, could be.

Dave Wylly:

If your health is good, if your doctors are in network, we like

Dave Wylly:

the advantage play later in life because what you'll find is

Dave Wylly:

supplements and drug programs, those premiums will increase

Dave Wylly:

through the years and there can become a tipping point where

Dave Wylly:

you've slowed down, you're not traveling as much, your doctors

Dave Wylly:

are all in the same network, you are much more stationary.

Dave Wylly:

And that's where an Advantage program could

Dave Wylly:

make a lot of sense.

Dave Wylly:

Your doctors are in network and the price of your supplement

Dave Wylly:

and drug program have spiraled up through the years.

Dave Wylly:

There can be a flipping point there.

Josh Bretl:

So that goes back to the, it's not

Josh Bretl:

a set it and forget it.

Josh Bretl:

It's something you need to look at as much as you can.

Dave Wylly:

Shouldn't be.

Dave Wylly:

Yeah.

Josh Bretl:

We have people who listen to this all over

Josh Bretl:

the country, and this is a question I didn't tell you

Josh Bretl:

I was going to answer or ask you, but is Medicare

Josh Bretl:

different state by state?

Dave Wylly:

Well, you get 47 states that do it the same

Dave Wylly:

way, and then you've got three that are wacky and do

Dave Wylly:

it a little bit differently.

Dave Wylly:

But for the most part, when you have Medicare running the

Dave Wylly:

show, it's a nationwide program.

Dave Wylly:

They're the network, they're the billers, they're the paperwork,

Dave Wylly:

they're all the follow-up.

Dave Wylly:

And that allows us a level of competition

Dave Wylly:

among all these companies that want your business.

Dave Wylly:

Wisconsin is a little bit different, Minnesota's a

Dave Wylly:

little different, and Rhode Island I think is the other one

Dave Wylly:

that's a little bit different.

Dave Wylly:

But for the most part, the platform is very standard

Dave Wylly:

across the United States.

Dave Wylly:

And once you understand the components, and

Dave Wylly:

that's a problem.

Dave Wylly:

People come to the world of Medicare, they've had

Dave Wylly:

an employer plan and all the decisions that went

Dave Wylly:

into their healthcare program were all dictated

Dave Wylly:

to them by the employer.

Dave Wylly:

It was a wonderful benefit, but they've not been used to the

Dave Wylly:

putting together a health plan.

Dave Wylly:

What's the network?

Dave Wylly:

What's the deductible?

Dave Wylly:

What's the co-payment?

Dave Wylly:

What's the drug benefit?

Dave Wylly:

What's the wellness benefit?

Dave Wylly:

The beauty of Medicare, which conversely can be the

Dave Wylly:

terror of Medicare, now you get to make those decisions.

Dave Wylly:

The beauty of it is it's totally customizable to what

Dave Wylly:

you want it to be, and it can be changed through the aging

Dave Wylly:

process if you'll stay engaged.

Josh Bretl:

That's funny.

Josh Bretl:

I've never thought about it that way.

Josh Bretl:

I talk about that all the time from a tax perspective and

Josh Bretl:

an investment perspective.

Josh Bretl:

Well, while people are working and just living their lives,

Josh Bretl:

there's not much control.

Josh Bretl:

There's not much decisions they have to make.

Josh Bretl:

The employer makes most of those decisions.

Josh Bretl:

You don't have control, but when you retire, all of a

Josh Bretl:

sudden you have control over when and how you're taxed.

Josh Bretl:

You have a lot more control over your investment options.

Josh Bretl:

But at the same is true with healthcare too.

Josh Bretl:

I never thought about that.

Dave Wylly:

Sure.

Dave Wylly:

It shouldn't be on autopilot.

Dave Wylly:

Just like you control your 401[k] withdrawals and that

Dave Wylly:

income level, you can control your healthcare to a large

Dave Wylly:

degree if you stay engaged.

Dave Wylly:

Every year, you get to change the drug program.

Dave Wylly:

I guess the biggest key to anything with regard

Dave Wylly:

to healthcare, the better your health, the more

Dave Wylly:

doors are open to you.

Dave Wylly:

So that's something we all need to get involved in.

Josh Bretl:

Yeah.

Josh Bretl:

Without a doubt.

Josh Bretl:

Let's talk about drug companies or the drug program

Josh Bretl:

for just a second here.

Josh Bretl:

Part D, and I know you've heard this, I've heard you

Josh Bretl:

say this over and over again.

Josh Bretl:

You need to look at it every single year.

Josh Bretl:

Why is that?

Dave Wylly:

You really should.

Dave Wylly:

The beauty of my industry, the cornerstone of Medicare

Dave Wylly:

is robust competition and complete transparency.

Dave Wylly:

So these companies, and there's many of them.

Dave Wylly:

For example, here in Illinois, we currently have 24 different

Dave Wylly:

drug programs to choose from.

Dave Wylly:

My God, why 24?

Dave Wylly:

No, no, no.

Dave Wylly:

26 would be better.

Dave Wylly:

28 would be better still because they're all focused in

Dave Wylly:

on winning your business, but kept on the rails by Medicare.

Dave Wylly:

Medicare shows them the drugs they can cover and they

Dave Wylly:

don't have to cover them all.

Dave Wylly:

So you have specialization, you have niches, whether it's

Dave Wylly:

very healthy guy that's taking generics or a very sickly person

Dave Wylly:

who needs very expensive drugs.

Dave Wylly:

But every year these guys sharpen their pencil.

Dave Wylly:

Every year they got to show us everything.

Dave Wylly:

medicare.gov a wonderful website.

Dave Wylly:

They have to show us their cards every year starting October 1st.

Dave Wylly:

You have the ability to utilize capitalism.

Dave Wylly:

Capitalism kept on the rails works.

Dave Wylly:

And we've seen over the last 15 years a dramatic decrease in

Dave Wylly:

the cost of the drug programs.

Dave Wylly:

But any drug program that you choose needs to be correct

Dave Wylly:

for your situation, for the pharmacy you want to use and

Dave Wylly:

for the meds you are taking.

Dave Wylly:

There's a lot of variables there, but when you understand

Dave Wylly:

how to do it really can make a lot of sense every

Dave Wylly:

year just to double check.

Josh Bretl:

And your medication could change every year.

Josh Bretl:

The cost of your medication could change.

Dave Wylly:

Constantly, we have drugs going generic.

Dave Wylly:

That's been a godsend for a lot of people.

Dave Wylly:

Crestor used to be a brand name drug.

Dave Wylly:

It was $110.

Dave Wylly:

Now, it's called Rosuvastatin Calcium.

Dave Wylly:

It's five bucks.

Dave Wylly:

But you might not know that if you didn't check on it.

Josh Bretl:

Yeah, that's crazy.

Josh Bretl:

Well, Dave, you are the most knowledgeable Medicare

Josh Bretl:

professional I've ever met.

And you have a very similar philosophy.
