Access Tusc was established as a healthcare/social service network in 2014 to assist patients with navigating the resources within Tuscarawas County. Using Community Health Workers to build rapport with patients/clients within their own home environment, and establish a plan of action surrounding ‘pathways’ that must be completed in order to move through their current state of need will connect services and clients.
There are 20 identified pathways according to the HUB model:

- Developmental referrals
- Developmental screening
- Education
- Employment
- Family planning
- Health insurance
- Housing
- Immunization referral
- Immunization screening
- Medical home
- Medical referral
- Medication assessment chart
- Medication assessment

- Smoking cessation
- Social service referral
CHW's will come meet a client wherever is comfortable to help get all women in need connected to the right services.
The Bridges to Wellness HUB program offers help to pregnant women on Medicaid who need help navigating the services in the county including transportation, health care coordination, dental, behavioral health services, education, and many more pathways brought to their home by a Community Health Worker. Our pilot program is 100% available to any women who are dealing with recent/current opiate addiction, regardless of their insurance.
