So now I'm going to explain exactly what Peyronie's disease

is, how it happened, why it lead to ED, and specifically how the

Speaker:

penis pump, used correctly, can be of help for the curvature. So

stay with me. This is the conversation most men never get

to have, and they need to.

Speaker:

So let me start with Peyronie's disease. What is it at the

Speaker:

tissue level? So inside the penis there is a layer. So think

Speaker:

of the penis muscle, and you have two on both sides that's

Speaker:

the corpus cavernosum, a wrap around the corpus cavernosum, is

Speaker:

a layer called the tunica albuginea. And think of it as an

Speaker:

outer sleeve of the penis muscle. Think of it like a

Speaker:

sausage, and you have the wrapping around the sausage, is

Speaker:

what contain the blood inside the muscle during an erection,

Speaker:

and it gives the penis the shape and the structure. So in a

Speaker:

healthy penis, the tunica albuginea is the elastic. It

Speaker:

stretches evenly on both sides as it fills with blood. And

Speaker:

during an erection, it create a straight, firm result. So

Speaker:

Peyronie's disease happens when scar tissue develop inside the

Speaker:

tunica albuginea. So think of it like a scar tissue that gets

Speaker:

developed on that wrapping, and it developed the scar tissue and

Speaker:

then called a plaque. And scar tissue does not stretch. So when

Speaker:

the rest of the tunica albuginea. stretches during an

Speaker:

erection, the side with the scar tissue or the plaque stays

Speaker:

rigid. So that means that the penis bend towards the scar like

Speaker:

a balloon that have the patch of tape on one side, the elastic

Speaker:

side expands, the tape side doesn't, and the whole thing

Speaker:

curve. Does that make sense?

Speaker:

So how common is Peyronie's disease? So Peyronie's disease

Speaker:

affects somewhere between six to 13% of men. Many researchers

Speaker:

believe the actual number is probably higher because men are

Speaker:

probably not talking about it or going to the doctor about it too

Speaker:

much. It's such a personal problem. In my clinic, I see it

Speaker:

constantly. Men would have this for years and quietly stop

Speaker:

having sex, and no one talk about it at all, and almost

Speaker:

every single one of them said the same thing. When I finally

Speaker:

name it, they thought, "Well, I wasn't the only one. Thank God,

Speaker:

it's not something weird. I don't have cancer." So you're

Speaker:

not the only one. So what's happening in your body has an

Speaker:

explanation and a treatment pathway, and it has an

Speaker:

explanation and a treatment path. Men always asked me, Dr

Speaker:

Anne, how did this happen? The other answer is, is one of three

Speaker:

ways.

Speaker:

One, acute trauma. It's a sudden injury to an erect penis during

Speaker:

sexual activity or maybe we even a fall, or maybe in an accident

Speaker:

that causes like little tears in the tunica albuginea. So the

Speaker:

body responds to this tear like as with any wound, it starts to

Speaker:

put scar tissue into it, and because of that, it forms a

Speaker:

rigid, inflexible scar.

Speaker:

Number two, repeated micro trauma. Many men with Peyronie's

Speaker:

disease cannot identify a single injury. That's because the

Speaker:

plaque can form years before the injury, maybe a little

Speaker:

aggressive sex, and most of the time, this happened in a cowboy

Speaker:

position, where the woman's on top or bending during an

Speaker:

erection or repeated friction, each one creating a tiny

Speaker:

inflammatory response that makes the body deposit more scar

Speaker:

tissue over time. Number three, this is something you may not

Speaker:

know, the connection that nobody talked about, and it's

Speaker:

critically important when a man has untreated ED, the penis

Speaker:

muscle is not receiving regular blood flow, so without blood

Speaker:

flow, the corpus cavernosum or the penis muscle along the side

Speaker:

here, begin to atrophy or shrink and stiffen. So as the tissue

Speaker:

become less elastic, scar tissue start to form within it. This is

Speaker:

why ED and Peyronie's are often found together. ED doesn't just

Speaker:

coexist with Peyronie's. In many cases, untreated ED is what

Speaker:

create the condition for Peyronie's to develop. There's

Speaker:

also a genetic component, so men with a condition called

Speaker:

dupuytren's contracture, where you have scar tissue that

Speaker:

happened on the fourth finger here, like a big scar tissue

Speaker:

that make your finger bend like this, that's called dupuytren's.

Speaker:

Well, if you have dupuytren's, you're more at risk to getting

Speaker:

Peyronie's as well, too. And age plays a big role. Testosterone

Speaker:

decline reduces the elasticity of the corpus cavernosum and

Speaker:

lower estrogen in men often coincide with lower testosterone

Speaker:

also affect the collagen quality. So the tunica

Speaker:

albuginea, or the wrapping, becomes less forgiving, so more

Speaker:

susceptible to scarring with each passing decade,

Speaker:

particularly after the age of 40.

Speaker:

So the scar forms in two phases. The first phase is acute phase,

Speaker:

where it's forming, it's hot, it's tenders, inflamed. This is

Speaker:

when you have pain with intercourse. Phase two is more

Speaker:

chronic. This is like 12 months plus, right where inflammation

Speaker:

has resolved. The plaque has become calcified, and is

Speaker:

stabilized and the curve is fixed. So this does not mean

Speaker:

it's untreatable. It just means that the curve is fixed at this

Speaker:

point.

Speaker:

cavernosum, or the penis muscle. And then there's the

psychological impact, right? The shame, the avoidance,

embarrassment and then the depression. And men who wait

Speaker:

often find their options have narrow and that your only option

Speaker:

is really surgery. So you have two choices. You can have the

Speaker:

injection, where they dissolve the collagen, or surgery. But

Speaker:

let's talk about something that you can do without the injection

Speaker:

that if you're starting to notice maybe low curvature, that

Speaker:

you can do on your own. And that's called the penis pump,

is not an overnight fix. Scar tissue does not dissolve in the

week. But it does remodel with consistent work, all right? But

soften, it can form. It become more pliable. This is what

if your curvature is more than 30 degrees, you probably need to

go a urologist and maybe consider having surgery. Pump

Speaker:

consistent pumping does to the penile plaque. Every time you

Speaker:

should be less than probably 25 degrees at this time. The main

Speaker:

thing, guys is that you're starting to see some curvature,

use the pump, you create a negative pressure that draw

blood into the penile tissue, and it stretches the corpus

may even reverse it. You don't want it to get it worse and

worse. So it's not magic. This is the biology responding to the

cavernosum, including the area where the scar tissue was

right input. Just think of it like that. I want you to know

that if you are having a more severe curvature, that's more

formed. This daily stretch put gentle, sustained traction on

than 30, then you probably need to look at getting surgically

corrected. But there is an option, if you're less than

the plaque. And over weeks and months, this mechanical traction
