Painful sex after 50 usually comes down to a massive drop in
Speaker:three specific hormones. I'm Dr. Anne Truong, and here is how to

Speaker:"Ma'am, why don't women want to have sex after the age of 50
Speaker:years? My woman also complained that having sex, she feels pain
Speaker:as she has wound. Please explain the reason." So women have ED
Speaker:like men, but it shows in very different ways. We don't have a
Speaker:penis, so there's no erection, you may say. A woman's penis is
Speaker:the clitoris, right? The clitoris, is her penis, it's a
Speaker:miniature penis. So for women, even in the 40s, mid 40s, start
Speaker:to undergo perimenopause, or the beginning of menopause. So, what
Speaker:happens with perimenopause, and even in women after 50, like
Speaker:your woman, is that hormone changes in her the same way
Speaker:hormone changes in you. So, in women, they have three hormones
Speaker:to deal with: estrogen, testosterone, and progesterone,
Speaker:and its balancing. Balancing of those three hormones. Believe it
Speaker:or not, the testosterone hormone is the most in her, followed by
Speaker:estrogen, and then progesterone. So, men have 100 times more
Speaker:testosterone than women, but women have 100 times more
Speaker:estrogen than men, and so that will give women and men the
Speaker:sexual characteristics. When women after 50, the first thing
Speaker:that drop is their testosterone, and then the testosterone will
Speaker:be responsible for her libido and her sexual arousal. And the
Speaker:second one that drop is progesterone, so that affects
Speaker:her mood and her sleep. Third one that dropped is her
Speaker:estrogen, which is involved with blood flow.
Speaker:So, what I'm trying to say is that for your woman, after 50,
Speaker:her hormone drop. So when her hormone drops, it affects the
Speaker:lubrication down there. So it affect the lubrication around
Speaker:the opening of her vagina, and also the lubrication around her
Speaker:clitoris, so it doesn't lubricate as well. And for a
Speaker:woman's vagina, if it doesn't lubricate well, it's not
Speaker:moisturized, the opening of the vagina actually comes sensitive,
Speaker:but sometimes will even tear. So sometimes it will tear, like at
Speaker:6o'clock. So the opening, because it's not lubricated,
Speaker:feels raw. It feels like sandpaper for her when she has
Speaker:sexual activity, especially during penetration. It feels
Speaker:like sandpaper. It feels raw, and sometimes it may even bleed
Speaker:around the opening of the vagina with penetration. So knowing
Speaker:that for women, women need longer foreplay, they need
Speaker:longer foreplay, they still need blood flow down there to
Speaker:increase arousal, to increase the nerve in order for her to
Speaker:get aroused and get an orgasm. But she only needs four times
Speaker:more blood flow. Versus the man, he needs 40 times, four zero,
Speaker:more blood flow down in the penis to get an erection. But
Speaker:women still need blood flow too. So, what the blood flow does is
Speaker:that it brings in more sensation, but it also will help
Speaker:with the lubrication as well. So, women need more time for
Speaker:foreplay, for more blood flow, even probably around 15 minutes
Speaker:or so. Okay? Some women are even longer. And when she did
Speaker:menopause, all her hormone drops, and that's why she's not
Speaker:in the mood most of the time. And sometimes when her hormone
Speaker:drops, her vagina changes, it sags a little bit more, the
Speaker:bladder sometimes will be irritated, sometimes she'll have
Speaker:urinary tract infection, or UTI, and the vagina tissue is not as
Speaker:moist, it's not as flexible, so intercourse become more
Speaker:uncomfortable.
Speaker:But that can be easily corrected with topical estrogen. So she
Speaker:can get some estrogen or estradiol from her doctor. It
Speaker:can even be a primary care doctor or her gynecologist.
Speaker:Estradiol, she can get estradiol cream that she can just put
Speaker:around her vagina, spotting on the opening of her vagina. And
Speaker:also around her lips area. That will help with the lubrication
Speaker:and that will help with healing of the vagina and it will help
Speaker:her tolerate intercourse. But she also needs to be on hormone
Speaker:replacement therapy. She needs to get her estrogen, she needs
Speaker:to get her testosterone, she needs to get her progesterone.
Speaker:So she can see a hormone specialist doctor, she can see
Speaker:her gynecologist. However, not all gynecologists know how to do
Speaker:hormones, or not as welcoming to hormone replacement therapy.
Speaker:However, that's changing, but again, that depends on the
Speaker:doctor and the willingness and comfort level to do hormone
Speaker:replacement therapy. Urologists do hormone replacement therapy
Speaker:as well, too, or you can just go to a hormone specialist doctor.
Speaker:Well, she's going to need some topical estrogen down there to
Speaker:lubricate. She needs to put it around her vaginal opening to
Speaker:lubricate, so that way she doesn't hurt. And literally,
Speaker:within like a week will be changed significantly within a
Speaker:week, and that intercourse will be a lot more comfortable for
Speaker:her. It's actually can be easily corrected, and for women, for
Speaker:them to be able to enjoy intercourse and have orgasm
Speaker:again, it's really about hormone, but also have a
Speaker:gynecologist look down there as well to see if there's any other
Speaker:thing going on, any other medical condition going on down
Speaker:there. There's some cases she may have lichen sclerosus. If
Speaker:she does have a uterus, make sure she gets pap smear every
Speaker:year, or every three to five years, depending on her risk
Speaker:factors. And so the main thing is, go see a doctor, ask for
Speaker:some topical estrogen. But there's also some over the
Speaker:counter lube as well, too, that she can use. There is one that
Speaker:is in our store called Bliss, and it lubricates well. There
Speaker:you go, it's called Bliss oil for men and women. It is
Speaker:cannabis infused oil, and it actually lubricate very well in
Speaker:the vaginal opening, and she should also put this on her
Speaker:clitoris as well, too. But I recommend this Bliss, and you

Speaker:I hope that helped, but let her know it can be correctable, it
Speaker:can be treated. She doesn't have to live with it, and in fact,
Speaker:for her it can be easily treated within a week, unlike ED. So for
Speaker:women it is particularly simple to reverse. It's just getting
Speaker:the word out there. So thank you for asking that question, and
Speaker:for me to answer that, because it may help more people than you
Speaker:think. Because a lot of women suffer silently. And this
Speaker:sometimes will actually affect your relationship too, because
Speaker:she's uncomfortable, she doesn't want to do it, and then you
Speaker:don't want to push her, but you didn't know that this is due to
Speaker:hormone, and then you don't want to push her, and then you avoid
Speaker:sex, she avoids sex, no one talking to each other, and if
Speaker:you're not getting an erection, if you're not debating, again,
Speaker:you may even put yourself at risk for atrophy of that penis,
Speaker:and even put yourself at risk to even get ED. Unless you use a
Speaker:penis pump, then it can really affect your relationship as
Speaker:well. I know relationship is more than just physical, but the
Speaker:physical part is also an important part of the whole
Speaker:relationship as well. So, just know for her it can be treated,
Speaker:she just need to know and go ask her doctor. Okay? And we're just
Speaker:not talking about sex, we're really talking about her health
Speaker:too. Because if she gets hormone replacement therapy with her
Speaker:estrogen, her testosterone, her progesterone, it will also
Speaker:protect her against Alzheimer's in the future, heart disease in
Speaker:the future, and also bladder issue down there in the future,
Speaker:as well. So it's not just getting hormone for sexual
Speaker:activity, it's really her getting hormone for her health
Speaker:and her quality of life.
